Understanding Blood Pressure
What Is Blood Pressure?
Blood pressure is the force of blood pushing against the walls of your arteries as your heart pumps blood. It is measured in millimeters of mercury (mmHg) and recorded as two numbers:
- Systolic (top number): The pressure in your arteries when your heart beats (contracts).
- Diastolic (bottom number): The pressure in your arteries when your heart rests between beats.
Both numbers are important. Elevated systolic pressure is a major risk factor for cardiovascular disease, especially in older adults. Elevated diastolic pressure is more common in younger adults.
AHA Blood Pressure Categories
The American Heart Association (AHA) defines the following categories for blood pressure in adults:
- Normal: Systolic < 120 mmHg and Diastolic < 80 mmHg — optimal range.
- Elevated: Systolic 120–129 mmHg and Diastolic < 80 mmHg — increased risk of developing hypertension.
- Hypertension Stage 1: Systolic 130–139 mmHg or Diastolic 80–89 mmHg — lifestyle changes and possibly medication.
- Hypertension Stage 2: Systolic ≥ 140 mmHg or Diastolic ≥ 90 mmHg — medical treatment recommended.
- Hypertensive Crisis: Systolic ≥ 180 mmHg and/or Diastolic ≥ 120 mmHg — seek immediate medical attention.
Why Is Blood Pressure Important?
High blood pressure (hypertension) is often called the "silent killer" because it typically has no symptoms but can cause serious health problems over time:
- Heart disease: Increased risk of heart attack, heart failure, and coronary artery disease.
- Stroke: High BP damages blood vessels in the brain, increasing stroke risk.
- Kidney damage: Can cause kidney failure requiring dialysis.
- Vision loss: Damages blood vessels in the eyes (retinopathy).
- Dementia: Increased risk of vascular dementia and cognitive decline.
- Aneurysms: Weakened blood vessels that can rupture.
Risk Factors for High Blood Pressure
- Age: Risk increases with age. Most people develop hypertension by age 65.
- Family history: Genetics play a significant role.
- Overweight/obesity: Excess weight increases vascular resistance.
- Physical inactivity: Sedentary lifestyle raises BP.
- High sodium diet: Excess salt causes fluid retention and increased BP.
- Alcohol and tobacco: Both raise blood pressure and damage blood vessels.
- Stress: Chronic stress elevates cortisol and blood pressure.
- Sleep apnea: Repeated breathing interruptions during sleep increase BP.
How to Lower Blood Pressure Naturally
The DASH diet (Dietary Approaches to Stop Hypertension) is clinically proven to lower blood pressure:
- Reduce sodium: Aim for less than 2,300 mg/day (about 1 teaspoon of salt).
- Eat more fruits and vegetables: Rich in potassium, magnesium, and fiber.
- Choose whole grains: Replace refined grains with whole grains.
- Include lean proteins: Fish, poultry, legumes, and nuts.
- Limit saturated fats: Reduce red meat and full-fat dairy.
- Exercise regularly: 150 minutes of moderate exercise per week.
- Manage stress: Meditation, deep breathing, or yoga.
- Moderate alcohol: Limit to 1 drink/day for women, 2 for men.
- Quit smoking: Tobacco raises blood pressure and damages arteries.
💡 Clinical Insight: The SPRINT trial (2015) published in the New England Journal of Medicine found that targeting a systolic BP of 120 mmHg (intensive treatment) significantly reduced cardiovascular events and all-cause mortality compared to the standard target of 140 mmHg in high-risk patients. Always discuss personalized BP targets with your healthcare provider.
How to Measure Blood Pressure Correctly
- Use a validated, calibrated blood pressure monitor.
- Sit quietly for 5 minutes before measuring — feet flat on the floor, back supported.
- Place the cuff on your bare arm, at heart level.
- Don't talk or move during the measurement.
- Take 2–3 readings, 1–2 minutes apart, and average them.
- Measure at the same time each day (preferably morning and evening).
- Avoid caffeine, exercise, and smoking 30 minutes before measurement.
When to See a Doctor
- Hypertensive Crisis (≥180/120): Seek emergency care immediately.
- Stage 2 Hypertension (≥140/90): Consult your doctor promptly.
- Stage 1 Hypertension (130-139/80-89): Discuss lifestyle changes and monitoring with your healthcare provider.
- Elevated BP (120-129/<80): Review lifestyle modifications.
- If you have diabetes, kidney disease, or heart disease: BP targets may be lower — follow your doctor's advice.
