The Mosteller Formula (Clinical Standard)

The Mosteller formula, published in the New England Journal of Medicine in 1987, is the most widely used BSA formula in clinical practice:

BSA (m²) = √(Height in cm × Weight in kg ÷ 3600)

Its simplicity and accuracy make it the preferred formula in most hospitals and clinical guidelines. It is also easy to calculate without a calculator, which is a significant advantage in clinical settings.

The DuBois Formula (Historical Standard)

The DuBois formula, published in 1916, is one of the earliest BSA equations:

BSA (m²) = 0.007184 × Height(cm)^0.725 × Weight(kg)^0.425

While still used in some clinical settings and research, it has largely been replaced by the Mosteller formula due to its simplicity. The DuBois formula tends to give slightly higher values for some individuals, particularly those with higher weight.

Average BSA Values

Based on population studies:

  • Adult men: Average BSA ≈ 1.9 m² (range: 1.6–2.5 m²)
  • Adult women: Average BSA ≈ 1.7 m² (range: 1.4–2.2 m²)
  • Children: Varies by age; BSA of a 10-year-old is approximately 1.1 m²
  • Infants: BSA of a newborn is approximately 0.2–0.25 m²

Clinical Applications of BSA

Chemotherapy dosing: Many cancer drugs (e.g., cisplatin, carboplatin, doxorubicin) are dosed based on BSA to minimize toxicity and optimize efficacy. BSA-based dosing accounts for inter-individual differences in drug distribution.

Cardiac output index: Cardiac Index (CI) = Cardiac Output ÷ BSA. A normal CI is 2.5–4.0 L/min/m². This allows comparison of cardiac function across patients of different sizes.

Glomerular filtration rate (GFR): eGFR is normalized to a standard BSA of 1.73 m². This allows comparison of renal function across individuals of different body sizes.
